You'll need the Layer Effects script for this tutorial!

Go ahead and make a gradient going down on your project, but make sure it's lighter to darker. My foreground color is #00FCFF, and my background color is #0000FF.
Image

Now grab your Paths tool, and make an arrow, then go to the tool options, and select selection from path.
Image
Image

Now select the background layer, and go to Edit/Copy. Now deselect the selection and view the project in full view right now so you can see everything, and then press CTRL+V to paste your image, but you should still have a floating selection. Now press Shift+CTRL+N to put our copied selection earlier in a new layer, and it should be centered right now if the arrow is not centered on your project then you didn't just copy the arrow, or you didn't paste it viewing the whole project.

Anyway select your Dodge/Burn tool, and make sure you have burn selected in the tools menu. Then burn the arrow a couple times to a result you like.
Image

Then go to Script-Fu/Layer Effects/Bevel and Emboss. Now change the style from Outer Bevel to Inner Bevel then change the Highlight Color from white to black (#000000), and then the Shadow Color from black to white (#FFFFFF).

Now merge the 2 new layers that were created with the layer that has the arrow, now duplicate the background layer, and merge the layer with the arrow into that duplicated background layer. Now rename that layer, and call it Download.
Image

Select your Background layer (it should be the layer with the gradient) and go to Filters/Map/Map Object. Change Map To from Plane to Box, and check transparent background.
Image

Now mess with the x/y coordinate planes for the rotation of how you want your box displayed.
Image

Now select the Download layer and apply the same filter.

NOTE: If you're image looks like this...
Image

Then press CTRL+Z to undo that filter, and then merge the layer into your background layer. Now apply the same filter again. Now if your image looks like this, you're good to move onto the next step.
Image

Now press CTRL+A to make a selection on our whole arrow box, and then select CTRL+C to copy it.

Now undo the filter, and undo when we merged the layer down by going to Edit/Undo or pressing CTRL+Z.

Now we should have our Download layer back press Delete, and delete the image in the layer, and then paste our arrow cube in the layer. Now grab your eraser tool, and remove the arrow on the top, and left side on the cube, and then merge the layer into the background layer.
Image

Now select the paths tool again, and make a selection on the top of the box like so.
Image

Now grab your Dodge/Burn tool (It should still be set to burn) and burn the top part of the download image.
Image

Now crop it so it's just the download box, and then go to Filters/Shadow/Drop Shadow. Leave it at it's default settings, and you're done!
