Many popular antivirus programs also analyse files to see if they contain any suspicous code. MD5 can be bypassed by changing a single byte, some smart viruses will do this.

for example , a while ago there was a member who was making tutorials on how to make worms in .NET wich u probably can't spot with hashes but there are ofcourse ways to make virusses with a file(like a picture) and command prompt...

You could change your program to work on the basis of looking for safe file hashes rather than known bad ones.
